AED Request Form
The Eastern Ohio AED Initiative is a non-profit program in partnership with the American Red Cross. The goal is to purchase and distribute 500 or more AEDs to law enforcement groups, fire departments, and at non-profit community buildings in Carroll, Harrison, and Tuscarawas counties in five years. Please fill out the requested information below and return it to the address below for consideration of an AED for your non-profit organization. The Eastern Ohio AED Initiative along with the American Red Cross, will be providing CPR/AED training for groups awarded an AED.
